South Gloucestershire Council (SGC) are working with service users and partners to implement a new Supported Living Framework.
Supported Living is one option available to service users, including those with a learning disability, mental health illness, autism and a physical and/or sensory impairment who are on a journey of development or recovery. It supports them to develop independent living skills and to live as independently as possible in their own home and local community.
The framework is about having the right contracting arrangements to ensure the services we buy on behalf of the Council to:
- Maximise individuals opportunities to reach their full potential
- Include goals in their support plans for them to progress or recover, so that they have clear outcomes to work towards and agreed ways of monitoring how they are doing
SGC want to co-produce the principles of the new Supported Living Framework with key partners including:
- service users
- carers
- providers
- social care practitioners
- commissioners
- brokers
SGC would like to invite anyone who has an interest in this work to 3 x 2 hour online sessions on 27th October, 24th November and 15th December.
The sessions are about hearing from you on what matters most to you as someone who uses or would like to use Supported Living. For all partners, how SGC can work better together to offer choices that will help individuals thrive and live their best lives.
This will be to agree the key principles and to come to a shared understanding to develop an effective framework for all types of supported living.
SGC propose to discuss all elements at the 1st session and 2nd sessions which will include:
- A common understanding of definitions
- Payment rates and methods
- Delivering outcomes and monitoring
