Sirona Learning Disability Liaison Nurses are working in partnership with North Bristol Trust staff to promote that each ward and department has a Learning Disability Champion.
The aim of the LD Champion is to raise awareness on the wards and departments of patients with Learning Disabilities accessing their services, to ensure that they receive a positive experience and the treatment they require with any reasonable adjustments.
The Learning Disabilities Liaison Nurses actively promote the hospital passport to assist the patient and staff.
Individual “social stories” and communication passports are encouraged and can be adapted for admission, treatment and procedures.
